FollowMyHealth (FMH) Patient Portal

Your secure online connection to the Tennessee Memory Disorders Foundation & Genesis Neuroscience Clinic

Beginning March 1, 2026, TMDF Clinic uses the FollowMyHealth (FMH) portal for all patient communications and access to medical records. FMH gives patients a simple, secure way to stay connected to their care team anytime.

How to Request Access

To get started, contact the TMDF Clinical Coordinator and request an electronic invitation.

For security, please be ready to confirm:

  • Full name of the patient

  • Date of birth

  • Mobile number

  • Email address

Your invitation will be sent to your preferred email. The document notes: “Please check the spam folder.”

What You Can Do in FollowMyHealth

FMH allows you to manage your health information 24/7 from any computer, smartphone, or tablet. According to the guide, patients can:

  • “Communicate securely with your physician”

  • “View test and lab results”

  • “Complete forms before your appointment”

  • “Schedule and manage appointments”

How to Connect Your Account

1. Sign In or Create an Account

When you receive your invitation email, click the link provided.

If you’re new to FMH, select Sign Up and Connect to create your username and password. If you already have an account, choose Sign in and add this connection to link Genesis Neuroscience Clinic.

2. Accept the Terms of Service

You’ll be asked to review and accept the FMH Terms of Use and Privacy Policy before continuing. The document states: “By clicking on ‘Accept’ you are agreeing to the Terms of Service above and the Privacy Policy.”

3. Enter Your Invite Code

Your invite code is usually your date of birth in MMDDYYYY format, unless your provider gave you a different code. Example from the guide: “July 9, 1985 would be 07091985.”

For proxy accounts (children or dependent adults), the code applies to the patient.

4. Request for Access

You’ll see a screen confirming your name and requesting permission to connect your health information to FMH. The guide shows: “I authorize Genesis Neuroscience Clinic to release such information… to my FollowMyHealth personal health record.”

Select Accept to continue.

5. Upload Health Record

FMH will begin retrieving your basic demographics and clinical information.

What You’ll See After Connecting

Once logged in, your FMH home screen may show:

  • New messages

  • New clinical items

  • Appointment history

  • Options to add mobile numbers or manage communication preferences

You can also access:

  • Messages

  • My Health

  • Wellness tools

  • Health Summary

  • Devices & Apps (to sync health data)
